Does removing EXIF change how the photo looks?

No. EXIF is a separate block of text-like data alongside the pixels. Removing it leaves the visible image identical, though the file gets very slightly smaller.

Do social media sites already strip EXIF?

Most large platforms do strip it on upload, but messaging apps, email attachments and cloud file links very often do not. If you're sending the original file rather than posting it, assume the metadata is still there.
